Insurance

FuncPhysio is an out-of-network provider of physical therapy. You can use insurance that has out-of-network coverage. However, reimbursement is dependent on your insurance plan’s out-of-network coverage.

This is based on your out-of-network deductible, coinsurance, and out-of-pocket maximum. Our staff would be happy to help look into your benefits and go over a detailed explanation with you.


Why does FuncPhyso not participate in the network with insurance companies?

In-network insurance companies will not reimburse Physical Therapy services for one full hour of one-on-one care. A therapist at a typical in-network physical therapy clinic has to reduce he/her treatment time to an average of only 15 minutes per session.

Let me explain. Before establishing FuncPhysio, I had worked in many different in-network clinics where I had to see 2-4 patients simultaneously in an open space. How much do you think a licensed physical therapist can spend time treating you directly?? Not much… Your treatment session could be >60 minutes. However, You may be passed to non-professional assistants such as “trainer,””technician”, or “assistant.” who help you to do self-exercise, hot/cold packs, E-stim and etc. And you are asked to come for the session 2-3 times per week!! You may think the treatment is cheap, but the cost of your time and traveling will add up.

Most importantly, a patient who requires proper treatment of one-on-one care cannot get quality care. At the same time, a physical therapist who wants to make patients better cannot provide the best of care.

This is the reason that FuncPhysio is not in-network with insurance companies.


What you need to prepare before your first visit

Once you made an appointment with us, you will receive an online intake form. Please fill it out the Initial Consultation Form and Consent the Therapy Form, which can be found at the link sent to you via email. Detailed information such as symptoms, medical history, and medications will be helpful to the therapist, so please be sure to fill out the form in advance. We would also appreciate it if you could upload your insurance card and a letter of referral from your doctor to the website so that we can complete the form in advance.

You can also fill out the form at the clinic on the day of your visit, but in that case, please arrive about 15 minutes early, which give us enough time to prepare your session.


Notes for initial consultation/follow-up session

We reserve a one-hour slot just for you. Please do not be late for your appointment, as it will shorten your examination and treatment time.

Please wear comfortable clothes, avoiding skirts, jeans, etc. You may move around even you come for your jaw pain. We have a shirt and T-shirt available for you in case you do not have one.

  1. Appointment Time: Your appointment is scheduled for a one-hour duration and each session is reserved to each patient. Please make every effort to arrive on time to ensure you receive a thorough examination and treatment.
  2. Attire: We recommend wearing comfortable clothing for your appointment. Avoid skirts or jeans. This is important even if you’re coming in for jaw pain, as you may need to move around. If you don’t have suitable clothing, we can provide you with a shirt or T-shirt.

What is a Physical Therapist (Physiotherapist)?

  • A Physical Therapist (physiotherapist) is a health professional who provides treatment of injuries, rehabilitation after surgery, improvement of body functions, physical adjustments, and injury prevention. You may wonder about the difference in physical therapists to osteopaths, chiropractors, massage therapists, and trainers. Often we take the similar approach but different thought processes. Most importantly, FuncPhysio provides a holistic view of treatment by integrating Easter and Western medicine.

I do not have insurance. Can I still receive treatment?

  • Yes, If you do not have insurance or do not have out-of-network coverage, FuncPhysio is happy to provide treatment for those who do not have insurance. You can receive treatment at your own cost. In that case, please contact the clinic directly for the treatment fee.

Do I need a referral letter from my doctor?

  • According to New York State regulations, a referral letter is not required. However, some insurance companies may require a medical certificate from a doctor.

What is the Deductible and Co-Insurance on an insurance policy?

  • Deductible: The amount you pay for covered health care services before your insurance plan starts to pay. The amount of the deductible depends on your insurance plan’s out-of-network coverage.
  • Co-Insurance: the amount you pay for a portion of your treatment after you reach deductible. The amount depends on your insurance plan’s out-of-network coverage.

Is there a cancellation fee?

  • We charge a $75 cancellation fee for the third or subsequent cancellations within 48 hours. We also charge a $75 cancellation fee for the second or subsequent no-shows. In order to provide the highest quality of care, FuncPhysio blocks one hour of therapist time just for you. We appreciate your understanding of the cancellation fee. We ask that you contact us as soon as possible to reschedule or cancel your appointment. **Special circumstances such as natural disasters are taken into consideration.
